At Thomas Family Medical Practice, we’re committed to helping our senior patients enjoy the best possible quality of life as they age. Our Over 75 Health Assessments are designed to provide a clear picture of your overall health, identify potential concerns early, and support your independence and wellbeing.
Most of the assessments are bulk-billed under Medicare and are available once every 12 months for eligible patients aged 75 and over. Please speak to our receptionist for details.
Aging comes with unique health challenges. Regular health assessments allow us to:
Detect changes in memory, mobility, and mood
Review medications and their interactions
Identify risks of falls, frailty, or chronic disease
Create or update care plans
Offer support for carers and family members
Your comprehensive health assessment would include:
Including chronic conditions, hospital visits, surgeries, and medications.
Checking for memory changes, mood disorders, or signs of cognitive decline.
Assessing balance, gait, and home safety.
Basic sensory testing to flag any deterioration.
Support for maintaining a healthy lifestyle.
Discussing isolation, carer needs, or aged care services.
Creating a tailored plan with follow-ups, referrals, or home support.
Our reception team is here to assist you with more information and to schedule your appointment. These appointments will also be available to book on HotDoc for your convenience.
The assessment is typically conducted in two parts:
A nurse or care coordinator gathers initial information and performs screenings.
A GP consultation follows, where your doctor reviews results, answers questions, and discusses the next steps.

Yes. If you have a valid Medicare card, the Over 75 Health Assessment is fully bulk billed, meaning there is no out-of-pocket cost for eligible patients. We still advise calling our reception and clarifying the details.
The assessment usually takes about 45 to 60 minutes, including time with our nurse and your GP. We recommend allowing a full hour to ensure a thorough review.
No fasting is required. Just bring your Medicare card, a list of current medications, and any recent hospital records or relevant documents.
It includes a review of your medical history, medications, mobility, memory, mental health, vision, hearing, nutrition, and social support needs. A personalised care plan may also be created.
Absolutely. We encourage patients to bring a trusted support person, especially if it helps with communication or care planning.
Your GP will explain the results and, if necessary, create a care plan or refer you to other health services. We may also book follow-ups or home support visits.
Medicare allows this assessment to be done once every 12 months. We recommend scheduling it annually as part of your proactive healthcare routine.
